Applying to the National Honor Society: What would be a good explanation for tardiness?

The candidacy form for the National Honor Society at my school requires an explanation for “excessive absences or tardiness,” and while I’m rarely absent (once a school year, if that), let’s just say I’m not the most punctual person.

What should I say in my explanation?
Better late than never?
Should I say that I’m working hard to change my ways?

My daughter got a letter in the mail from this society. Is it a scam or is it for real? They want us to send them $60 for enrollment and I don’t want to be scammed. I’m concerned that my daughter will be disappointed and that her information will be used for something else. Has anyone heard or this society? Do you know anything about it?
